Package/Gift Certificate Purchases
The purpose of offering packages is to provide a treatment price break to our clients.
All packages and gift certificates will expire ONE YEAR from the purchase date and are non-transferable.

Treatment Cancellations
The scheduled time has been held open for you with a technician standing by ready to serve
you. If you have failed to give our staff 24 hours notice of cancellation or re-scheduling
of your appointment, our staff reserves the right to charge you one treatment. There are
NO exceptions to this policy. Because of the 24 hour cancellation requirement, a credit card
may be required when scheduling services, even if payment will be made in cash, check or by
gift certificate.

Late Arrivals
Scheduling is planned to allow the correct time for each service. Please be on time or several
minutes early to insure full treatment time. If you are late for your appointment time, your
treatment will be shortened so we finish on time for the next patient. The fee for the treatment
will remain the same.

Payment for Services
Payment for services must be rendered prior to any treatment taking place. Kindly pay the
receptionist for treatments before the technician or physician performs the treatment. All
purchases made for products and/or treatments and deposits made for surgical procedures are
NON-REFUNDABLE. Please review your surgical agreement for other payment details. There will be a $25 Returned Check Fee for any check that is returned, regardless
of the reason.
